Prepare for your visit

What to bring to your tax appointment

Bringing the right paperwork the first time means one visit instead of two. If you are missing something, come anyway; we will tell you what to send later.

Individuals and families

Personal returns

Identification

  • Photo ID for you and your spouse
  • Social Security cards or ITIN letters for everyone on the return, including dependents
  • Last year’s federal, Ohio, and city returns if you are new to us
  • Bank routing and account numbers for direct deposit

Income

  • W-2s from every employer
  • 1099 forms: interest (1099-INT), dividends (1099-DIV), retirement (1099-R), Social Security (SSA-1099), contract work (1099-NEC), unemployment (1099-G), brokerage statements (1099-B)
  • Records of rental income and expenses
  • Gambling winnings (W-2G) and any other income

Deductions and credits

  • Mortgage interest (1098) and property tax bills
  • Tuition statements (1098-T) and student loan interest (1098-E)
  • Child care provider name, address, tax ID, and amount paid
  • Health insurance forms (1095-A, B, or C)
  • Charitable contribution receipts
  • Medical expenses if they were significant this year
  • Estimated tax payments you made during the year

Life changes

  • Marriage, divorce, a new child, a move, a new job, or a home purchase or sale: bring the paperwork and we will sort out what matters
  • Any letter from the IRS, the Ohio Department of Taxation, or a city tax department

Owners and self-employed

Business returns

Records

  • Year-end bank and credit card statements for every business account
  • Income records: sales reports, invoices, 1099s received, merchant processor statements (1099-K)
  • Expense records by category, or the bookkeeping file if you keep one
  • Receipts for equipment, vehicles, or property purchased this year
  • Mileage log for business use of a vehicle
  • Home office square footage and household expenses if you work from home

Payroll and contractors

  • Payroll reports and the quarterly and annual filings (941s, 940, W-3)
  • Names, addresses, and tax IDs of contractors paid $600 or more (we prepare the 1099s)

Entity paperwork

  • Prior-year business return if you are new to us
  • Articles of organization, operating agreement, or S corporation election if the business is new
  • Loan statements showing interest paid

If we keep your books, most of this is already in hand. Bring the year-end statements and anything new.
